Job Summary:

We are looking for a dedicated Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Physical Therapist with LHC Group, you will be responsible for assessing and evaluating patient care needs, developing treatment plans, and implementing interventions to improve patient outcomes.

Responsibilities:
  • Evaluate patient functional status and physical therapy needs
  • Develop and implement treatment plans to improve patient outcomes
  • Provide physical therapy interventions to relieve pain, develop and restore function, and maintain maximum functioning
  • Communicate with patients, families, and healthcare teams to ensure effective care coordination
Requirements:
  • Current Physical Therapy licensure in the state of New Jersey
  • Current CPR certification
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
