Join Our Team as a Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Ocean County. As a home health physical therapist, you will work with adult and geriatric clients on a per-visit basis, providing rehabilitative care and skilled nursing services to help them remain at home.

Responsibilities:

  • Make home visits to clients in designated geographic territories.
  • Perform diagnostic tests and measurements, such as mobility and range of joints, transfer status, stability, and safety assessments.
  • Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
  • Continually assess and revise the care plan, participating in multidisciplinary care planning as needed.
  • Educate and instruct clients, family members, or representatives on rehabilitative care and activities necessary for independent living.
  • Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.

Qualifications:

  • A current New Jersey Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
