Job Summary:

516 Visiting Nurse Association of Wisconsin, Inc. is seeking a skilled Home Health RN Case Manager to join our team in the Northeast Milwaukee Area. As a Home Health RN Case Manager, you will be responsible for providing high-quality patient care and coordinating services to ensure the best possible outcomes for our patients.

Key Responsibilities:

  • Assume responsibility for caseloads consisting of patients in specialty areas, providing comprehensive care and support to meet their unique needs.
  • Conduct thorough initial assessments to gather physical, social, and environmental data, and establish plans of care in accordance with physician orders and patient needs.
  • Provide direct patient care, adjusting nursing care processes as needed, and identifying problems, modifying care plans, and evaluating outcomes within an appropriate time frame.
  • Communicate effectively with physicians, family members, and other healthcare professionals to ensure seamless care coordination and patient satisfaction.
  • Facilitate discharge planning, utilizing resources to ensure continuity of care after discharge, and participating in developing discharge plans with patients, families, physicians, social workers, and other staff members as appropriate.
  • Participate in chart audits and continuous quality improvement activities, identifying opportunities for process improvement and contributing to the enhancement of our services.
  • Maintain accurate and up-to-date clinical and patient records, adhering to agency, state, and federal guidelines.

Requirements:

  • Registered Nurse license issued by the state in which you practice.
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) within 6 months of hire.
  • A valid driver's license issued by the Division of Motor Vehicles or the state of Illinois.
  • Associate's Degree in Nursing.
  • Typically requires 3 years of experience in clinical nursing.

  • Excellent communication and interpersonal skills, with the ability to work with diverse clients and their families.
  • Strong organizational, analytical, and problem-solving skills, with the ability to work independently and as part of a team.
  • Proficiency in clinical skills, with the ability to work under direction and make sound judgments.
  • Ability to operate necessary equipment, such as FAX, computer, copier, and telephone system.
  • Ability to assess data reflecting patient status and interpret information needed to identify patient requirements.
