POSITION TITLE: Advanced Practice Provider

DEPARTMENT: Grand River Health Clinics

REPORTS TO: Administrative Director of Clinic Operations

BASIC FUNCTION: Evaluates patient needs, formulates care plans, and delivers direct healthcare services within the Grand River Health Clinic environment.

SALARY INFORMATION:
  • $63.03 Based on Experience
  • Shift differentials may apply for eligible roles.
  • Bonus (If Eligible)
RESPONSIBILITIES:
  • Conducts patient assessments, performs thorough physical evaluations, and documents comprehensive medical histories, progress notes, and treatment plans.
  • Takes necessary actions to ensure appropriate care for illnesses and injuries, including prevention, diagnosis, treatment, and recovery.
  • May prescribe medications and order diagnostic evaluations. Provides guidance to patients regarding ongoing care.
  • Collaborates with Physicians and other interdisciplinary care team members.
  • Orders, interprets, and records results of various clinical tests, communicating findings to other Providers and departments as necessary.
  • Facilitates patient education initiatives.
  • Prescribes medications and recommends alternative therapeutic interventions.
  • In addition to clinical responsibilities, engages in quality assurance planning, initiatives, and improvement projects.
  • Participates in meetings and serves on committees as assigned.
  • Utilizes extensive experience and judgment to achieve objectives.
  • Operates under the supervision of the designated Physician.
  • Performs a variety of tasks and additional duties as assigned.
QUALIFICATIONS:
  • Graduation from an accredited Physician Assistant program is required.
  • Active Colorado Physician Assistant licensure is mandatory.
  • National Physician Assistant certification through NCCPA is preferred.
  • A Master's Degree in a relevant specialty is advantageous.
  • ACLS and PALS certifications may be required within six months of employment based on admitting privileges.
  • Required certifications and licenses must be maintained in current status throughout employment.
  • Preferably, at least two years of professional experience in a clinical or hospital environment.
