Job Summary

We are seeking a skilled and compassionate Caregiver Coordinator to join our team at Brio Living Services. As a Caregiver Coordinator, you will play a vital role in providing non-certified, participant-focused, non-clinical home care supports as directed by our interdisciplinary team.

Key Responsibilities
  • Supervise and Support Participants: Provide one-on-one supervision and support to assigned participants, ensuring their physical and emotional needs are met.
  • Facilitate Activities and Redirect: Facilitate diversional activities and redirect participants as needed to promote engagement and well-being.
  • Light Home Maintenance: Conduct light home making tasks, including dishes, sweeping, and general tidying, to maintain a safe and comfortable living environment.
  • Meal Preparation: Prepare meals for participants as necessary, promoting healthy eating habits and social interaction.
  • Transportation and Accompaniment: Ride along with participants on the bus when needed, providing transportation to their home, PACE Center, or external visits.
  • Day Center Coverage: Provide coverage in the Day Center when requested, ensuring a seamless and supportive experience for participants.
Requirements
  • Reliable Transportation: Possess reliable transportation and a valid MI driver's license.
  • Experience Working with Seniors: Have one year of experience working with the frail elderly, long-term care population, or end-of-life care, with a strong understanding of their unique needs and challenges.
