At Liberty Healthcare and Rehabilitation Services, our team provides a wonderful rehab experience for patients and their families by being responsive and individualizing treatment plans. Our multidisciplinary team is committed to progressive treatment modalities to address a variety of diseases and impairments in an in-house therapy setting with local management support.
Key Responsibilities:
- Work collaboratively with the multidisciplinary team to provide exceptional care to patients and their families.
- Develop and implement individualized treatment plans tailored to meet the unique needs of each patient.
- Utilize evidence-based practices and progressive treatment modalities to achieve optimal patient outcomes.
Requirements:
- Education: Must be a graduate of an accredited occupational therapy program.
- Certificate/License: Must be licensed in the state where the skilled nursing facility (SNF) is located.
- Experience: Previous rehabilitation SNF experience is preferred.
