Job Title: Registered Nurse - Home Health

Eden Health is a leading provider of home health services, and we are seeking a skilled Registered Nurse to join our team.

Job Summary:
  • Provide admission, case management, and follow-up skilled nursing visits for Home Health patients.
  • Administer on-going care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
  • Implement patient care plans in conjunction with patients and families to assist them in achieving optimal resolution of needs/problems.

Responsibilities:

  • Coordinate and oversee the work of Home Health Aides and provide written instructions that reflect current plans of care.
  • Report and document patient care/condition/progress to patient's physician and Clinical Manager on a continuous basis.
  • Prepare and complete necessary clinical documentation, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.

Requirements:

  • Graduate of an accredited nursing program.
  • Current licensed Registered Nurse (RN) in the state of practice.
  • Experience as an RN with one of the following preferred: one year of experience in an acute-care setting within the last 24 months or six months of Home Health experience within the last 12 months.
