The Medication Therapy Management Specialist plays a crucial role in optimizing medication therapy for patients through comprehensive reviews, education, and collaboration with healthcare teams.

Key Responsibilities:
  • Conduct thorough medication reviews and assessments to identify potential drug-related problems.
  • Develop and implement individualized medication therapy plans in collaboration with patients and healthcare providers.
  • Provide patient education on medication usage, adherence, and lifestyle modifications.
  • Monitor patient progress and adjust therapy based on clinical outcomes.
  • Document and report medication-related interventions and outcomes.
Requirements
  • Doctor of Pharmacy (Pharm.D.) degree with training in medication therapy management.
  • Board certification in medication therapy management (BCMTMS or equivalent) preferred.
  • Strong analytical and clinical skills.
  • Effective communication and interpersonal skills for patient and provider interactions.
