Job Title: Medication Therapy Management Specialist

At destinationone Consulting, we are seeking a highly skilled Medication Therapy Management Specialist to join our team. This role is critical in optimizing medication therapy for patients through comprehensive reviews, education, and collaboration with healthcare teams.

Key Responsibilities:
  • Conduct thorough medication reviews and assessments to identify potential drug-related problems.
  • Develop and implement individualized medication therapy plans in collaboration with patients and healthcare providers.
  • Provide patient education on medication usage, adherence, and lifestyle modifications.
  • Monitor patient progress and adjust therapy based on clinical outcomes.
  • Document and report medication-related interventions and outcomes.
Requirements:
  • Doctor of Pharmacy (Pharm.D.) degree with training in medication therapy management.
  • Board certification in medication therapy management (BCMTMS or equivalent) preferred.
  • Strong analytical and clinical skills.
  • Effective communication and interpersonal skills for patient and provider interactions.
