Job Summary

The Occupational Therapist in our Home Health team is responsible for assessing and evaluating patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. Based on this assessment, the Occupational Therapist helps determine a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

Key Responsibilities
  • Evaluate patient functional status, body systems, and occupational therapy needs
  • Consult with physicians to develop therapy plans of care
  • Observe, record, and report patient responses to treatment and changes in condition
  • Instruct and inform healthcare team members, patients, and caregivers about plan of care, functional limitations, and progress
  • Visit patients within the service area and submit progress notes according to LHC policy
  • Complete patient evaluations and develop OT plans of care within state-specific guidelines
  • Participate in staff conferences and committees as requested
  • May supervise home health aides and OTAs according to LHC policy and state regulations
Requirements
  • Current Occupational Therapy licensure in the state
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle

Schedule flexibility is required to ensure patient care needs are met. Ability to participate in weekend rotations and potential availability on observed holidays is necessary.
