We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at The Village at Willow Crossing. As a LPN, you will play a vital role in delivering high-quality nursing care to our residents in a warm and welcoming environment.

About the Role

As a LPN, your primary responsibility will be to assess, communicate, and document residents' status. You will also observe and report any significant changes in resident behavior and health to the Resident Care Director, physician, family, and other relevant parties.

Key Responsibilities
  • Accurately assess and document residents' status
  • Communicate effectively with residents, families, and healthcare teams
  • Direct and supervise the daily work assignments of Resident Care Assistants
  • Mentor and guide Lead Resident Care Assistants in their roles
  • Coordinate care needs with community providers through effective case management
  • Balance service demands with supporting resident independence
  • Recognize and prevent elder abuse, neglect, and exploitation
Requirements
  • Current state license as an LPN and CPR certification
  • Graduate of an approved LPN program (per state requirements)
  • Minimum of 1 to 2 years of experience working as a nurse preferred
  • Previous experience working with the elderly in a paid or volunteer position is highly desirable
