Job Summary

This position plays a critical role in developing and executing pharmacy care management services to ensure the medication needs of Essentia Health's Accountable Care Organization (ACO) contract enrollees are met in a safe, efficacious, and cost-effective manner.

Key Responsibilities
  • Develop and execute pharmacy care management services in collaboration with other disciplines within Essentia Health.
  • Strategically develop an Essentia Health ambulatory formulary.
  • Share accountability with the Population Care Management department for pharmacy spend goals for Essentia's ACO contracts.
Requirements
  • Minimum of 1 year or equivalent pharmacist experience.
  • Experience with managed care, formulary management, medication therapy management, and geriatrics are preferred.
  • Graduation from a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE) or a program outside the U.S. that qualifies the individual to practice in the jurisdiction.
  • Licensure as a Registered Pharmacist by the Minnesota State Board of Pharmacy.
  • Licensure as a Registered Pharmacist by Wisconsin and North Dakota State Board of Pharmacies within 6 months of hire.
  • AMCP/ASHP pharmacy practice residency preferred.
