Job Summary

Region 1 Inc is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team. As a Home Health LPN, you will provide high-quality patient care in the comfort of their own homes, working closely with our Registered Nurses and other healthcare professionals to deliver exceptional patient outcomes.

Responsibilities

• Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
• Conduct thorough assessments of patients' physical and emotional needs, developing and implementing personalized care plans to meet their unique requirements.
• Collaborate with interdisciplinary teams to coordinate patient care, ensuring seamless transitions and optimal outcomes.
• Maintain accurate and detailed records of patient interactions, treatments, and progress, adhering to regulatory requirements and agency policies.
• Participate in ongoing education and training to stay current with best practices, new technologies, and emerging trends in home health care.

Requirements

• Current LPN licensure in the state of practice.
• Minimum one year of clinical nursing experience, preferably in an acute care setting.
• Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
• Ability to travel to patient homes, with a valid driver's license and reliable transportation.
• CPR certification and basic life support skills.
• Ability to lift up to 50 pounds, bend, stoop, and move with ease in patient homes.

Working Environment

• Direct patient care: 80%
• Travel: 15%
• Office: 5%
