We are seeking a skilled and compassionate Restorative Nurse to join our team at Joliet Terrace. As a Restorative Nurse, you will play a vital role in coordinating care for our residents, assisting them in maintaining their highest level of physical functioning, and working closely with our therapy team to help patients reach their clinical goals.
Key Responsibilities- Coordinate restorative care for residents, including assisting with daily living activities and providing support with mobility and transfers.
- Work closely with our therapy team to develop and implement individualized care plans that promote resident independence and well-being.
- Monitor resident progress and adjust care plans as needed to ensure optimal outcomes.
- Collaborate with other healthcare professionals to ensure seamless care transitions and effective communication.
- Licensed Practical Nurse (LPN) license in IL.
- 2+ years' experience in a SNF setting as a Restorative Nurse is preferred.
- The restorative/rehabilitation certification is ideal.
- Electronic medical records experience is helpful (Point Click Care or PCC is ideal).
