Job Summary

We are seeking an experienced LVAD Coordinator RN to join our team. As a key member of our healthcare team, you will be responsible for coordinating patient care and managing evaluations, admissions, and discharges for LVAD patients.

In this role, you will have the opportunity to work with a variety of patients and families, providing high-quality care and support. You will also have the chance to develop your skills and knowledge in a dynamic and supportive environment.

Responsibilities
  • Coordinate patient care and manage evaluations, admissions, and discharges for LVAD patients
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure seamless care transitions
  • Provide education and support to patients and families
Requirements

To be considered for this role, you must have a minimum of 2 years of experience in a relevant field and a current RN license. A BLS certification is also required within 30 days of hire.
