Position Overview:
As a Registered Nurse Care Coordinator, you will work Monday through Friday from 8 AM to 5 PM. Your primary responsibilities will include administering nursing care to adult patients in their homes, collaborating with an interdisciplinary team, families, and referring agencies, and overseeing the coordination of care.

Reporting Structure:
You will report directly to the Managing Director of Hospice.


Qualifications:
Education:
Must be a graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.

Licensure:
A current Texas State license as a Registered Nurse and a valid Texas Driver's License are required.

Experience:
A minimum of one year of experience as a Registered Nurse in a clinical care environment is necessary, with hospice experience preferred. Current CPR certification is required. Proficiency in reading and writing English is essential.

Skills:
Nursing skills that align with generally accepted standards of practice, along with strong interpersonal abilities. Current CPR certification is required. Proficiency in reading and writing English is essential.

Transportation:
Reliable transportation and valid auto liability insurance are mandatory.

Work Environment:

This role involves working in patients' homes under various conditions, with potential exposure to blood, bodily fluids, and infectious diseases. Flexibility in scheduling and local travel is required, along with some exposure to inclement weather and PRN emergency calls. Consent or declination for Hepatitis and proof of a TB test are necessary.

Physical and Mental Demands:
The position requires prolonged standing and walking, with the ability to lift up to 50 lbs and assist patients. You will need to work under some stressful conditions to meet deadlines and address patient needs, making quick decisions and resource allocations to cater to individualized psychosocial requirements. Hand-eye coordination and manual dexterity are also essential.


Education Requirements:
An Associate's degree or higher in Nursing is required.

Licenses & Certifications:
A valid RN License is mandatory.
