About the Opportunity

We are seeking an experienced pharmacist to join our team as a Transition of Care specialist at a 250-bed hospital in the Northern Columbus suburbs.

This role offers an exciting opportunity to make a meaningful impact on patient care and outcomes, working collaboratively with healthcare professionals to ensure seamless transitions between care settings.

Key Responsibilities
  • Function as a drug specialist and therapy consultant, promoting optimal medication use and minimizing adverse events;
  • Pounds, packages, labels, and dispenses drugs and intravenous admixture solutions with precision and accuracy;
  • Reviews and confirms the accuracy of work performed by pharmacy technicians, ensuring high-quality patient care;
  • Reviews prescribed drug therapy for appropriateness, effectiveness, and safety, taking into account patient-specific factors such as age, disease state, and clinical status;
  • Performs quality assurance, drug use evaluation, and adverse reactions reporting activities to continuously improve patient care and safety;
What We Offer

As a valued member of our team, you will enjoy a competitive compensation package, starting at $65.00 per hour.

Our ideal candidate will have a PharmD degree, be licensed as a pharmacist in Ohio, and possess at least one year of relevant experience.
