We are seeking a skilled Case Manager to join our team at Windsor Healthcare Communities. As a Case Manager, you will play a critical role in coordinating patient care to ensure optimal outcomes, cost-effectiveness, and communication.
Key Responsibilities:- Conduct pre-admission assessments to ensure clinically appropriate admissions in accordance with Federal, State, and Company requirements.
- Coordinate patient care to meet patient, payor, and Company needs for patient outcome, cost, and communication.
- Determine resource utilization specific to patient care needs, outcome expectations, payor, and Company requirements.
- Participate in negotiating coverage as assigned.
- Ensure exchange of essential information, i.e., payor feedback, clinical outcomes, therapy utilization for the provision of quality patient service.
- Attend all Center or Company required inservices and meetings.
- Maintain all documentation necessary for quality case management according to Federal, State, and Company requirements.
- Current California RN license.
- Strong administrative and organizational skills.
- Minimum 2 years experience in case management.
- Bachelors degree in health care field preferred.
