Key Responsibilities:
- Clinical Evaluations: Perform thorough assessments of patients with life-limiting conditions.
- Advanced Symptom Management: Apply advanced expertise to effectively manage complex symptoms.
- Medication Oversight: Prescribe and modify medications to alleviate symptoms and reduce side effects.
- Collaborative Care: Lead and engage in multidisciplinary care team discussions to deliver holistic treatment.
- Patient and Family Guidance: Provide education and support to patients and families regarding their health conditions and treatment pathways.
Qualifications:
- Master's degree in nursing.
- Valid state nursing license and Controlled Substance authorization.
- Active DEA License without restrictions.
- A minimum of one year of experience in home care or hospice settings.
- Certification in Hospice and Palliative Care is preferred.
- Familiarity with the local medical community and the ability to enhance the hospice's reputation.
- Capability to collaborate effectively with the patient's primary healthcare provider to ensure optimal medical care.
