Case Manager

Lutheran Services Florida (LSF) is seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in supporting the well-being of youth and families in need.

The Case Manager will be responsible for conducting initial intake interviews, gathering information, and developing service plans to ensure the timely reunification of youth with their families. You will work closely with youth, sponsors, and stakeholders to provide support and guidance throughout the process.

• Conduct initial intake interviews to gather information and assess the needs of youth and families.
• Develop and implement service plans to ensure the timely reunification of youth with their families.
• Provide support and guidance to youth, sponsors, and stakeholders throughout the reunification process.
• Document all actions taken and contacts with youth, sponsors, and stakeholders in the form of progress notes.
• Collaborate with other departments and stakeholders to ensure the provision of comprehensive services to youth and families.

• Bachelor's degree in Social Work, Psychology, Human Services, or a related field.
• 1 year of experience working with adolescents or in the youth services field.
• Must be computer literate with working knowledge of Microsoft Windows and Microsoft Office.
• Must possess a valid state driver's license and be eligible to drive to facilitate program services.
• Bilingual English/Spanish or English/Creole required.
