Atrium Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team in a Skilled Nursing setting. As a Nursing Care Specialist, you will play a vital role in providing high-quality patient care and collaborating with our interdisciplinary team to achieve exceptional patient outcomes.
Key Responsibilities- Assessment and Planning: Participate in determining nursing care needs based on collection and interpretation of data relevant to the patient's health status.
- Recognition and Intervention: Recognize and determine the need for immediate intervention, and take appropriate action to ensure patient safety.
- Documentation and Communication: Document patient observations, treatments, and responses to treatment, and communicate effectively with the healthcare team to ensure seamless care coordination.
- Collaboration and Teamwork: Work collaboratively with Registered Nurses, physicians, and other healthcare professionals to develop and implement individualized patient care plans.
- Physical and Emotional Demands: Meet the physical and emotional demands of the role, including lifting, standing, and walking, and maintaining a calm and composed demeanor in high-pressure situations.
- Education: Graduate from an accredited School of Practical Nursing.
- Certifications: Basic Life Support (BLS) certification required.
- Licensure: Current LPN license or temporary license as a Licensed Practical Nurse Petitioner in the state where you work and reside.
