As a Care Partner, you will play a vital role in delivering high-quality care and services to our residents. Your primary responsibilities will include:
- Providing personal care and support to residents in accordance with their individualized service plans
- Communicating effectively with medical professionals, families, and other team members to ensure seamless care coordination
- Participating in meal service, including serving, clearing, and cleaning the dining room
- Documenting resident care and services in accordance with company policies and regulatory requirements

Requirements:
- High School Diploma or equivalent required
