As a Patient Care Coordinator, you will have the opportunity to work with our Nursing Education team in a 1-year new graduate program.

Responsibilities:
  • Provide high-quality direct and indirect patient-centered care using the nursing process.
  • Coordinate patient care by working collaboratively with the interdisciplinary team.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
  • Develop and implement individualized treatment plans that address the unique needs of each patient.
  • Prioritize tasks efficiently and make sound judgments in emergency situations.

You will be responsible for creating a safe and therapeutic environment, promoting positive patient outcomes, and documenting the nursing process in a timely manner. Your ability to think critically, communicate effectively, and work autonomously will be essential in this role.

Requirements:
  • A current Registered Nurse license with the state of Connecticut.
  • Certification in Cardio Pulmonary Resuscitation (CPR) upon hire and maintenance during employment.
  • Training in PMT and de-escalation techniques, with certification within 6 months of hire.
  • Basic computer skills and proficiency in electronic health records.

In exchange for your expertise, we offer a competitive salary range of $65,000 - $85,000 per year, based on experience and qualifications. Additional benefits include:
