Patient Care Coordinator

At National Lutheran Communities & Services, we're seeking a dedicated Licensed Practical Nurse to join our team and help us deliver exceptional healthcare services to our patients.

This position requires a blend of clinical skills, empathy, and strong teamwork. You'll be part of creating a warm, supportive environment for our patients, where individual strengths and experiences are valued and utilized.
Responsibilities- Provide compassionate, patient-centered care tailored to individual needs
- Assist in developing and implementing unique patient care plans
- Administer medications and treatments as prescribed
- Monitor and record patients' conditions and vital signs
- Closely collaborate with RNs, physicians, and other healthcare professionals
- Educate patients and their families on health maintenance and disease prevention
- Ensure a clean, safe, and comfortable environment for patients
- Participate in team meetings and contribute ideas to improve patient care processes
- Maintain accurate and detailed patient records
- Respond promptly and professionally to patient needs and concerns
To succeed in this role, you'll need:
- A current LPN license in good standing
- A minimum of 1 year of clinical experience preferred
- Strong clinical skills and knowledge of current healthcare practices
- Excellent communication and interpersonal skills
- A compassionate nature with a genuine interest in patient well-being
- The ability to work effectively in a fast-paced, team-oriented environment
- Proficiency in electronic health record systems
- Flexibility to work various shifts, including some weekends and holidays
- BLS certification required
