Job Title: Hospice Admissions Coordinator Specialist

We are seeking a dedicated and experienced professional to join our team as a Hospice Admissions Coordinator. The ideal candidate will work collaboratively with our referral unit to ensure seamless patient admissions to hospice care.

Duties and Responsibilities:
  • Perform all intake duties for new patients, including demographic data collection, insurance verification, and accurate EMR management.
  • Collect and analyze data from referral sources, family members, and facilities to complete pre-admission information.
  • Conduct initial welcome phone calls with patients and families to ensure a smooth transition.
  • Obtain necessary information for referrals and admissions to Health Care Connectors Hospice.
  • Schedule admissions visits efficiently and effectively.
  • Facilitate DME and pharmacy orders for new admissions.
  • Communicate and collaborate with the Hospice Team on all admissions.
  • Assist in preparing materials needed for admission visits.
  • Facilitate safe transitions to hospice care by coordinating services with referral sources and outside agencies.

Requirements:
  • The ideal candidate must have experience in hospice care.
  • Prior experience with Epic and Care Port is preferred.
  • Knowledge of HIPAA and Medicare regulations is essential.
  • A Bachelor's degree is preferred but not required.
