Job Description
We are seeking an experienced Care Coordinator to join our team of dedicated professionals at Mental Health Partners. In this role, you will play a vital part in delivering exceptional care to adults experiencing mental illness and substance use issues in Boulder and Broomfield Counties.

Key Responsibilities
This role involves conducting intake assessments, developing treatment plans, and providing case management services to individuals struggling with mental health conditions. You will work collaboratively with our interdisciplinary team to ensure seamless service delivery and promote positive outcomes for our clients. Your expertise in building relationships with clients, advocating for their needs, and connecting them with community resources will be invaluable in achieving our mission.

Qualifications and Requirements
To succeed in this role, you must have a strong academic background in a relevant field (Bachelor's Degree in Psychology, Social Work, Counseling, or related) or equivalent experience. Excellent communication and interpersonal skills are essential, as well as the ability to work effectively in a fast-paced environment. A valid Colorado driver's license, safe driving record, and willingness to travel within Boulder and Broomfield Counties are required.
