The Case Manager is a key member of the patient care team, responsible for implementing and coordinating patient care plans to ensure safe, timely, and efficient care. This role requires strong communication and problem-solving skills, with a focus on continuous improvement and cost-effectiveness.
Key Responsibilities:
- Develop and implement patient care plans in collaboration with the interdisciplinary care team
- Coordinate patient care activities, including pre-admission, placement, and follow-up care
- Collaborate with patients, families, and external healthcare providers to ensure seamless care transitions
- Monitor and evaluate patient outcomes, identifying opportunities for improvement
- Communicate effectively with internal and external stakeholders to resolve issues and promote cooperation
Requirements:
- BSN required
- Minimum of five years of relevant clinical experience
- Prior case management experience highly desirable
- Current licensure as a registered nurse by the State of Maryland
