Key Responsibilities:

  • Develop and implement patient care plans in collaboration with the interdisciplinary care team
  • Coordinate patient care activities, including pre-admission, placement, and follow-up care
  • Collaborate with patients, families, and external healthcare providers to ensure seamless care transitions
  • Monitor and evaluate patient outcomes, identifying opportunities for improvement
  • Communicate effectively with internal and external stakeholders to resolve issues and promote cooperation

Requirements:

  • BSN required
  • Minimum of five years of relevant clinical experience
  • Prior case management experience highly desirable
  • Current licensure as a registered nurse by the State of Maryland
