Job Summary

We are seeking a skilled Case Manager to join our team at FamilyTIES of SC LLC. The successful candidate will be responsible for coordinating Community Long Term Services (CLTC) to assigned participants, providing case management services face-to-face and over the phone, and developing relationships with providers and division leaders to ensure clients receive the most appropriate and effective care possible.

Key Responsibilities:

  • Assess, plan, implement, monitor, and evaluate actions required to meet the client's health and human services needs
  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
  • Handle case assignments, draft service plans, review case progress, and determine case closure
  • Help clients achieve wellness and autonomy
  • Facilitate multiple care aspects, including case coordination, information sharing, and advocacy

Requirements:

  • Proven work experience in case management, including nursing, medical, mental health, care management, or a related field
  • Excellent knowledge of case management principles, healthcare management, and reimbursement
  • Previous experience with psychological aspects of care
  • Effective communication skills
  • Excellent organizational and time management skills
  • Professional and technical skills
  • Problem-solving skills and ability to multi-task
  • Compassionate with teamwork skills

Preferred Qualifications:

  • Bachelor's degree from an accredited college or university in a health or human services field
  • Documentation of at least two years' experience providing case management services
  • Certified Case Manager (CCM) certification

Work Environment:

  • Part-time position
  • Monday to Friday schedule
  • $25.00-$30.00 per productivity
