Key Responsibilities:- Coordinate care for assigned patients, developing and implementing comprehensive care plans.
- Conduct on-site supervisory visits to assess patient, family, and clinical caregiver needs.
- Take on-call duties as assigned, providing timely and responsive patient care.
- Uphold and promote adherence to CHAP standards, ensuring delivery of high-quality and compliant nursing services.
- Contribute to retention initiatives by fostering a supportive work environment and promoting caregiver satisfaction.
- Associate, Bachelor's, or Master's degree in Nursing from an accredited program.
- Current RN licensure in designated state(s).
- Valid Driver's license and acceptable MVR.
- Experience in home care or clinical leadership roles preferred.
