We are seeking a dedicated Patient Care Supervisor for our Hospice services at Heart of Hospice, a part of LHC Group. In this role, you will be instrumental in fostering a culture of compassion, inclusivity, and trust, while building meaningful relationships with patients, their families, colleagues, and the communities we serve.
Key Responsibilities- Leadership: Inspire and motivate your team to deliver outstanding patient care.
- Staff Support: Ensure all team members receive the necessary orientation and support to excel in their roles.
- Communication: Act as a liaison among staff, patients, and the healthcare community.
This is a full-time position, with an estimated distribution of 70% office work and 30% fieldwork. A minimum of 4 years of experience as a Registered Nurse is preferred.
Role ExpectationsThe Patient Care Supervisor in Hospice will assist the Executive Administrator/Executive Director/Director of Clinical Operations in overseeing all patient care activities, ensuring the provision of safe and effective care that complies with nursing standards, regulatory requirements, and payer criteria.
You will be responsible for:
- Adhering to hospice policies and procedures, as well as local, state, and federal regulations.
- Promoting quality care and patient safety while guiding your team.
- Engaging in comprehensive problem-solving and complaint resolution.
Additionally, you will conduct in-service training for staff to enhance patient health and safety, familiarize them with regulatory standards, and address any educational needs identified.
QualificationsTo be considered for this role, candidates must possess:
- A current RN license in the state of practice and at least one year of clinical experience.
- Current CPR certification.
- A valid driver's license, vehicle insurance, and reliable transportation.
Additional State-Specific Requirements for Louisiana:
A minimum of two years of full-time experience as an RN is required. Alternatively, two years of full-time clinical experience in hospice care as a licensed practical nurse may be substituted for the required RN experience.
