Job Summary
The Mental Health Support Worker II position involves providing direct support and case management services to adults with serious mental illness, emphasizing independence, stability, and overall well-being.
Duties and Responsibilities
- Act as a primary support and case manager for assigned clients, facilitating access to essential services and resources.
- Conduct comprehensive assessments to identify individual needs and develop personalized plans, incorporating input from clients, family members, and healthcare providers.
- Coordinate care with multidisciplinary teams, leveraging expertise and resources to address complex client needs.
- Provide emotional support and guidance to help clients manage crisis situations and make informed decisions about their treatment.
- Maintain accurate records and reports, ensuring timely submission and adherence to organizational policies and procedures.
Requirements
- A minimum of one year's experience in mental health or community services, with a focus on direct support and case management.
- A high school diploma or equivalent, or an associate's degree in a related field; bachelor's degree preferred.
- Valid driver's license and reliable transportation, as travel may be required.
- Ability to work independently and as part of a team, maintaining confidentiality and adhering to HIPAA guidelines.
- Familiarity with crisis intervention techniques and knowledge of local resources and support services.
