Position Overview

We are seeking an experienced Adult Mental Health Case Manager II to join our team, providing direct support and case management services to adults with serious mental illness.

Responsibilities

  • Develop and implement individualized plans to promote independence, stability, and overall well-being.
  • Collaborate with multidisciplinary teams to address complex client needs, leveraging expertise and resources to drive positive outcomes.
  • Provide emotional support and guidance to help clients manage crisis situations and make informed decisions about their treatment.
  • Maintain accurate records and reports, ensuring timely submission and adherence to organizational policies and procedures.
  • Engage with clients, family members, and healthcare providers to ensure seamless coordination of care and promote optimal health outcomes.

Requirements

  • A minimum of two years' experience in mental health or community services, with a focus on direct support and case management.
  • A high school diploma or equivalent, or an associate's degree in a related field; bachelor's degree preferred.
  • Valid driver's license and reliable transportation, as travel may be required.
  • Ability to work independently and as part of a team, maintaining confidentiality and adhering to HIPAA guidelines.
  • Familiarity with crisis intervention techniques and knowledge of local resources and support services.
