Hospice Care Specialist

We are seeking a compassionate and skilled Hospice Nurse to join our team at Red Rock Hospice. As a Hospice Nurse, you will provide high-quality patient care and support to individuals with terminal illnesses and their families.
Key Responsibilities:
- Provide individualized care to patients in accordance with physician orders and established company policies.
- Assist in maintaining a safe and comfortable environment for patients, ensuring their rights are respected and protected.
- Document patient care and progress in a timely and accurate manner, adhering to company standards and regulations.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Administer medications and treatments as prescribed by physicians, monitoring patient responses and reporting any concerns.
- Conduct thorough assessments of patients' physical, emotional, and social needs, identifying areas for improvement and developing strategies for care.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Participate in ongoing education and training to maintain expertise in hospice care and stay current with industry developments.
Requirements:
- Current LPN or LVN license in good standing.
- One to two years of experience in hospice care or a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassion, empathy, and a commitment to delivering exceptional patient care.
