We are seeking a skilled Clinician/Social Worker to join our team on a PRN basis. As a Clinician/Social Worker, you will provide psycho-social assessments, individual and family therapies, and develop treatment plans for our patients.
Responsibilities- Conduct psycho-social assessments and develop treatment plans for patients
- Provide individual and family therapies to patients
- Carry a caseload and maintain regular contact with patients' referral sources and families
- Participate in unit activities and lead special focus process groups/educational groups
- Provide timely and accurate documentation
- Participate in referral development activities and performance improvement committees
- LPC/LCSW licensure preferred
- Minimum of two years experience in direct patient care
- MS degree in counseling, psychology, or related social services field
- Ability to work weekends and holidays as needed
