Job Summary

We are seeking a skilled Clinician/Social Worker to join our team on a PRN basis. As a Clinician/Social Worker, you will provide psycho-social assessments, individual and family therapies, and develop treatment plans for our patients.

Responsibilities
  • Conduct psycho-social assessments and develop treatment plans for patients
  • Provide individual and family therapies to patients
  • Carry a caseload and maintain regular contact with patients' referral sources and families
  • Participate in unit activities and lead special focus process groups/educational groups
  • Provide timely and accurate documentation
  • Participate in referral development activities and performance improvement committees
Requirements
  • LPC/LCSW licensure preferred
  • Minimum of two years experience in direct patient care
  • MS degree in counseling, psychology, or related social services field
  • Ability to work weekends and holidays as needed
