The Family Health Center is dedicated to providing excellent primary and behavioral health services to our patients and community partners. We strive to build relationships with our patients and community partners to offer comprehensive care.

Position Summary

Social workers provide direct service to clients with everyday life issues, complications, and problems, including incidents of neglect, abuse, or domestic violence, or with mental health and substance abuse challenges.

They provide clinical services to pediatric and adolescent clients experiencing mental illness, substance use disorder, dually diagnosed, and mentally ill/developmentally disabled.

This includes individual, family, group counseling, and psychiatric consultations. Social workers collaborate with clients to assess their goals and adjust them as needed.

When appropriate, they may offer clients and patients outside referrals for support groups or other mental health professionals.

If needed, social workers may also work with clients or patients to secure government intervention or assistance or respond to moments of crises.

We are seeking a Licensed Clinical Social Worker, LCSW, LMHC, or LMFT to join our team. We offer a $5,000 Sign on bonus.


Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:
  • Completes psychiatric evaluations.
  • Assists individual clients, families, and children to utilize appropriate community resources.
  • Assist residents and families with social, emotional, and family issues.
  • Develops and reviews clients' individualized treatment plans using person-centered treatment planning.
  • Performs individual, family, and group therapy.
  • Identify and address psychosocial needs of patient and family.
  • Reaches productivity standards set for clinical role.
  • Attends and participates in clinical staff meetings, group supervision, quality improvement projects, and supervision with the Operations Manager.
Minimum Qualifications (Knowledge, Skills, and Abilities)

Bachelor's in behavioral sciences, counseling, social sciences, or equivalent experience.

Master of Social Work, Mental Health Counseling, or Marriage and Family Therapy from an accredited Institution and Licensed as a LCSW by IPLA.

A natural leader and advocate.

Familiar with Diagnostic and Statistical Manual of Mental Disorders.

Collaborate with others in a professional manner.

Possesses sound critical thinking skills and can assist patients/clients with problem solving.

Understand HIPAA laws and protocols.


Pay Range: $66,000 - $72,000 per year Family Health Center
