About the Role:

As a Home Health Occupational Therapist at CenterWell Home Health, you will play a vital role in helping patients manage chronic conditions or recover from injury, illness, or surgery. Our care teams work together to provide personalized, comprehensive home care that addresses the physical, emotional, and social well-being of our patients.

Key Responsibilities:

  • Assess and develop therapeutic retraining programs to improve patients' daily living and work-related skills.
  • Administer and interpret diagnostic and prognostic tests to assist physicians in evaluating patient function.
  • Collaborate with healthcare teams to develop and revise treatment plans.
  • Provide therapeutic treatment and instruction to patients to improve strength, coordination, range-of-motion, and function.
  • Review and expand retraining programs to meet patient goals and rehabilitation program guidelines.
  • Consult with vocational team members to develop vocationally oriented plans.
  • Report patient status and progress to healthcare teams and family members.
  • Participate in care coordination and discharge planning activities.
  • Design community reintegration activities to assist patients in physical reconditioning and psychological adjustment.
  • Recommend and design special adaptive equipment to improve patient environments or participation in rehabilitation programs.
  • Train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills.
  • Coordinate and supervise Occupational Therapy Assistant and Home Health Aide performance.
  • Accurately document patients' care observations, interventions, and evaluation.

Requirements:

  • Degree from an accredited Occupational Therapy Program.
  • A minimum of six months of occupational therapy experience preferred.
  • Home Health experience a plus.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Good organizational and communication skills.
  • Valid driver's license, auto insurance, and reliable transportation.
