Job Summary

We are seeking a skilled Behavioral Health Case Manager to join our team. As a key member of our healthcare team, you will be responsible for coordinating and completing timely assessments, planning, implementation, and evaluation of discharge plans and care transitions across the continuum of care.

Key Responsibilities
  • Monitor daily census and assignment to ensure all patients are assessed for case management needs, including care coordination, transition, and discharge planning.
  • Conduct initial assessments within 72 hours to identify psychosocial strengths, weaknesses, community living supports present and needed.
  • Develop and implement discharge plans that create a safe environment for crisis contacts and community living.
  • Interface with the healthcare team to determine multidisciplinary assessments of discharge planning needs and provide information regarding support system/family function.
  • Maintain timely documentation of assessment findings, discharge arrangements, and actions taken according to departmental guidelines.
Requirements
  • Master's degree in social work from an accredited school of social work.
  • Current licensure as a licensed clinical social worker (LCSW) by the NC Social Work Certification and Licensure Board.
  • 3 years of relevant experience in adult/mental health.
