Hospice RN Case Manager Job Summary

We are seeking a compassionate and skilled Hospice RN Case Manager to join our team in Charleston, SC. As a Hospice RN Case Manager, you will play a vital role in providing exceptional care to patients and families in their homes.

Key Responsibilities:
  • Maintain a specified case management load, ensuring timely and effective care coordination.
  • Conduct comprehensive assessments of patients and families to determine hospice needs, including physical and psychosocial evaluations.
  • Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and families.
  • Coordinate care among IDG members and external healthcare providers to ensure seamless delivery of services.
  • Evaluate and coordinate the need for services, including inpatient, respite, and continuous care.
  • Supervise and direct services provided by LPNs, Hospice Aides, and Homemakers.
Requirements:
  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or evidence of current clinical knowledge.
  • One (1) year of hospice care experience.
  • CPR certification.
