Job Title: Registered Nurse - Home Care
Job Summary: We are seeking a skilled Registered Nurse to provide one-on-one nursing care in the home to critical, complex clients focusing on trach and/or vent patients of all ages.
Responsibilities:
- Provide high-quality, patient-centered care in the home setting
- Collaborate with patients, families, and healthcare teams to develop and implement individualized care plans
- Conduct thorough assessments, develop and implement care plans, and evaluate patient progress
- Administer medications, perform treatments, and provide wound care as needed
- Document patient information, progress, and outcomes in a timely and accurate manner
Requirements:
- Current RN license
- CPR certification (no online classes accepted)
- TB skin test (current within last 12 months)

Qualifications:
- CPR
- RN License
