Job Summary:

We are seeking a skilled Community Care Worker I to join our team at the Texas Department of Aging & Disability Services. As a Community Care Worker I, you will play a vital role in determining the needs and eligibility for Community Care for the Aged and Disabled (CCAD) services.

Key Responsibilities:
  • Conduct home visits and interviews with individuals who are older or have a disability to assess their needs and provide information and referral to other resources.
  • Determine eligibility and functional needs through reviews and assessments.
  • Obtain, verify, and calculate income and resources to determine financial eligibility for Title XX programs.
  • Maintain accurate and up-to-date case records and enter information in an automated system.
  • Monitor services to ensure they are meeting the individual's needs and develop service plans accordingly.
  • Prepare reports and communicate with individuals, families, and other stakeholders as needed.
Requirements:
  • Ability to establish and maintain effective relationships with individuals, families, and colleagues.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of resources and services for older adults and individuals with disabilities.
