Job Description

We are seeking a Licensed Practical Nurse (LPN) to join our team in providing individualized goal-directed nursing care to patients in their homes. As an LPN, you will work under the supervision of an RN or Clinical Manager to implement medical plans of care, nursing interventions, and procedures to ensure the safe and effective quality of care for patients.

Responsibilities
  • Provide quality care to assigned patients
  • Carry out the plan of care for each patient, ensuring that the patient/care givers are included in the care plan process
  • Ensure documentation is completed, communicating via a note the activities and needed actions of each visit to the RN
  • Implement medical plan of care, nursing interventions, and procedures as necessary to ensure the safe effective quality of care for patients
  • Evaluate the effectiveness of the implemented plan of care, making recommendations to the case manager as needed
  • Maintain timely and accurate documentation of nursing care provided to ensure the effective integration of information for use by the health care team
  • Perform patient and care giver education to ensure informed patient decisions can be made
  • Appropriately identify inter-disciplinary needs of the patient and document patient needs accordingly
  • Carry out visits in a time-effective manner, taking action to ensure that visits are not combined with routine visits whenever necessary
  • Transmit patient incident reports to the QI Specialist in a timely manner
  • Coordinate a wide variety of patient care with physicians, providers, team members, vendors, etc. in support of the recommended plan of care
Qualifications
  • Licensed Practical Nurses (LPN) license
  • Prior clinical acute care nursing experience required, prior home care experience preferred
  • Valid Driver's License, Vehicle and Insurance, ability to successfully pass motor vehicle record check, child abuse, criminal record check, and adult abuse check required
