The Client Support Specialist will deliver comprehensive case management services to individuals qualifying under the Missouri Division of Developmental Disabilities criteria, facilitating access to essential medical, social, educational, and other necessary services.

This is a full-time position eligible for benefits, operating Monday through Friday.

KEY RESPONSIBILITIES

  • Conduct thorough assessments and regular reassessments of individual needs to identify requirements for medical, educational, social, or other services.
  • Develop and periodically update a personalized care plan tailored to individual needs. Monitor the execution of the care plan to continually evaluate the individual's status, available service options, community engagement, strengths, needs, and progress towards objectives.
  • Assist individuals and their families in locating, accessing, and coordinating services and resources necessary for the eligible individual to obtain required services to meet identified needs and achieve established goals.
  • Complete all mandated documentation in accordance with agency policies, including but not limited to progress notes, individualized care plans, and reviews.
  • Engage in team meetings to review consumer cases concerning eligibility, support plans, progress, and potential modifications to the service program.
  • Travel to clients' residences and service locations to conduct assessments and monitoring as needed.
  • Perform additional duties as assigned.

    Required qualifications include a Bachelor's degree or a Nursing Diploma/Associate degree in nursing.
    (*A degree in a human service-related field is preferred; RN license is required if a nursing degree is held.)
  • Experience (either lived or trained) with individuals with developmental disabilities is required if the Bachelor's degree is not in a human service-related field.
  • A valid driver's license and agency-specified minimum automobile insurance coverage are mandatory.
