We are seeking a skilled Public Health Nurse to join our team at the Georgia Department of Public Health. As a Public Health Nurse, you will play a critical role in promoting and protecting the health and safety of the residents of Cobb and Douglas Counties.
Key Responsibilities- Assess clients with chronic medical conditions and develop plans of care to optimize healthy outcomes.
- Provide care coordination services and education to clients and their families.
- Consult and coordinate care with internal and external health care professionals.
- Bachelor's degree in nursing from an accredited college or university.
- One year of experience in public health nursing.
- Current Georgia license to practice as a Registered Professional Nurse.
- CPR certification.
- Bilingual English/Spanish.
- Experience with case management, children and adolescents with chronic medical conditions and special healthcare needs.
General office environment and in-home client visits.
Salary and Benefits$36.00/hour, up to 29 hours per week, no benefits.
