We are seeking a skilled Nurse Leader/Supervisor to join our team at RiverWoods Exeter. This is a dynamic role that requires strong leadership and clinical skills to manage and coordinate the care of residents.
Key Responsibilities- Manage and Coordinate Resident Care: Utilize the nursing process to assess, plan, and implement individualized care plans for residents.
- Collaborate with Team Members: Work with the interdisciplinary team to formulate and update resident care plans, and participate in Resident Care Plan Meetings.
- Assess and Document Resident Status: Monitor and document changes in residents' physical or mental status, and report findings to the primary care physician and treatment team.
- Provide Leadership and Direction: Supervise direct care partners, prepare daily assignments, and delegate workload appropriately.
- Establish Positive Relationships: Foster open, direct communication with team members, residents, and family members to ensure a positive and supportive environment.
- Administer Medications and Treatments: Obtain, administer, and document medications and treatments given to residents, following professional guidelines and Nursing department policies.
- Nursing Degree or Diploma: Must possess a minimum of a Nursing Degree or Diploma from an accredited college, university, or nursing school.
- Long Term Care Experience: Long term care facility experience and specialized knowledge of nursing care of the elderly and rehabilitative nursing is preferred.
- Certifications: Must hold a nursing license within the State of New Hampshire, and maintain CEU's annually, CPR certification, and Resident Assessment Tool certification.
