Job Summary

We are seeking a Hospice Care Specialist to join our team in this rewarding role. As a Hospice Care Specialist, you will be responsible for providing direct patient care, working closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient. Your compassion, empathy, and dedication to delivering high-quality care will make a significant difference in the lives of our patients and their families.

Responsibilities

- Provide direct patient care, including administering medications, performing vital signs, and documenting patient information

- Develop and implement individualized care plans in collaboration with our interdisciplinary team

- Conduct regular assessments to identify changes in patient condition and adjust care plans accordingly

- Maintain open communication with patients, families, and caregivers to ensure seamless coordination of care

- Collaborate with our medical staff to address patient concerns and coordinate referrals to specialists as needed

Requirements

- Current certification as a Certified Nursing Assistant (CNA) or equivalent experience

- 6 months of patient care experience in hospice, home health, ICU, geriatric, or oncology

- Strong communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams

- Ability to maintain confidentiality and handle sensitive patient information with discretion

- Availability to work flexible hours, including evenings, weekends, and holidays
