We are seeking a highly skilled and experienced Family Nurse Practitioner to join our integrated healthcare team at Olesky Associates, Inc. as a Primary Care Provider in an outpatient clinic setting.
Key Responsibilities:- Provide patient care services under the supervision of staff physicians and perform medical case management responsibilities.
- Perform annual exams and sick visits, ordering appropriate diagnostic tests and evaluating results, and establishing presumptive diagnosis and developing treatment plans.
- Maintain the follow-up of abnormal labs and work in cooperation with other team members to assure appropriate care, compliance, and treatment.
- Educate patients about diagnostic testing, procedures, medications, nutrition, and healthy living.
- Triage and return patient inquiry phone calls and follow-up as needed.
- Maintain and document patient charts as appropriate.
- PA License
