Key Responsibilities- Conduct thorough assessments of patients to determine their home care needs, including physical assessments and histories of current and previous illnesses.
- Develop and implement individualized care plans to meet the unique needs of each patient, incorporating therapeutic, preventative, and rehabilitative nursing actions.
- Administer medications and treatments as prescribed by physicians, and provide direct patient care as defined by the State Nurse Practice Act.
- Counsel patients and their families on meeting nursing and related needs, and provide health care instructions as appropriate.
- Identify discharge planning needs and implement plans prior to patient discharge.
- Act as a case manager when assigned, coordinating patient care for assigned caseloads.
- Associate or bachelor's degree in nursing, with one to two years of recent acute care experience in an institutional setting. Two years of home health care experience preferred.
- Current licensure as a Registered Nurse in the state.
- Current BLS certification.
- Licensed driver with an automobile in good working order and insured in accordance with the organization's requirements.
- Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
- Prolonged or considerable walking and standing, ability to lift, position, or transfer patients, and ability to lift supplies and equipment.
- Visual acuity and hearing to perform required skills.
- A stable and supportive work environment with a team of experienced healthcare professionals.
