Job Title: Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Potomac Healthcare Solutions. As a Clinical Pharmacist, you will play a critical role in providing high-quality patient care and contributing to the success of our healthcare services.

Job Summary:

The Clinical Pharmacist will be responsible for assessing patient responses to drug therapy, planning and implementing medication regimens, and monitoring patient outcomes. You will work closely with medical staff to ensure accurate and effective medication management.

Key Responsibilities:
  • Assess patient responses to drug therapy and plan medication regimens
  • Monitor patient outcomes and adjust medication regimens as needed
  • Collaborate with medical staff to ensure accurate and effective medication management
  • Order and evaluate laboratory tests to evaluate drug therapy effects and outcomes
  • Administer prescription and non-prescription medications according to established agreements and protocols
  • Assess metabolic needs and order therapeutic enteral or parenteral nutrition products
  • Consult with healthcare providers to discuss patient treatment needs and options
  • Review patient profiles to monitor medication therapy and ensure accuracy of pharmaceutical products
  • Provide medication and healthcare-related training to clinical staff
  • Document significant medication interactions and pharmacy interactions with prescribers
  • Conduct Adverse Drug Reaction (ADR) reporting in accordance with The Joint Commission (TJC) or the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC) standards
Requirements:
  • Doctorate Degree in Pharmacy
  • Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE)
  • Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy
  • Minimum of one year of professional pharmacy experience within the last two years
  • Current, full, active, and unrestricted license to practice as a pharmacist
  • BLS CPR certification
  • Must be a US Citizen
