Key Responsibilities:
- Coordinate case management activities with an interdisciplinary healthcare team to ensure effective service delivery to patients. Attend compliance training and adhere to organizational standards of conduct, policies, and procedures. Focus on customer service to enhance patient satisfaction. Conduct initial and comprehensive patient assessments and actively seek feedback for continuous improvement.
- Initiate and maintain communication with physicians and other team members, evaluate patients, and provide family-centered health education. Deliver direct patient care, assess outcomes, and adjust care plans as necessary to ensure optimal patient health. Collaborate with the healthcare team to establish care plans based on physician orders and patient needs.
- Participate in case conferences and team meetings, recognizing limitations and seeking assistance to meet performance goals. Complete all required documentation accurately and promptly in the electronic system while adhering to agency and regulatory standards.
- Exercise independent judgment in addressing patient care needs and communicate effectively with physicians. Identify and engage in process improvement initiatives that enhance service quality. Participate in weekend and on-call coverage as per guidelines.
- Demonstrate the knowledge and skills necessary to provide age-appropriate care for patients across the lifespan. Understand growth and development principles and assess patient data to meet individual care requirements.
- Possess a degree from an accredited nursing program. Previous experience in acute care, skilled care, home health, or hospice care is preferred but not required. A BSN is preferred.
- Establish and maintain positive relationships with healthcare professionals, staff, and patients/families. Work effectively in a complex environment, managing multiple priorities and specialized equipment. Exhibit strong clinical judgment, critical thinking, and problem-solving skills related to patient care. Maintain physical stamina for frequent walking, standing, lifting, and positioning of patients.
