About the Role

We are seeking a highly skilled and compassionate RN Patient Care Manager to join our team at Heartland Hospice. As a key member of our clinical operations team, you will be responsible for overseeing the day-to-day operations of our hospice sites, ensuring the provision of high-quality care to our patients, and supporting the Branch Director in their duties.

Key Responsibilities
  • Site Management: Manage the site in the absence of the Executive Director, Administrator, or Senior/Executive Patient Care Manager, ensuring seamless operations and high-quality patient care.
  • Clinical Oversight: Oversee clinical and administrative functions, including recordkeeping, to ensure compliance with regulatory guidelines and managed care principles.
  • Patient Care: Participate in on-call nursing schedules and provide direct skilled bedside nursing care in patients' homes, responding to phone referrals and inquiries in a timely, professional, and compassionate manner.
  • Staff Supervision: Supervise and manage staff, providing guidance and support to ensure excellent patient care and outcomes.
Requirements
  • Education: Registered Nurse with a minimum of Associate's Degree in Nursing required from an accredited school of nursing. Bachelor's Degree in Nursing preferred.
  • Experience: Three years' experience in direct patient care in a clinical setting required. Minimum one year of management/supervisory experience required. Two or more years' experience in hospice or home care required.
  • Licenses and Certifications: Current license to practice nursing in the state of requested employment. CPR Certification. Current automobile insurance and valid driver's license.

  • Sensitivity and Compassion: Sensitivity to feelings of the terminally ill and their families before and after patient's death. Ability to demonstrate flexibility and perform a variety of tasks without loss of efficiency or composure.
  • Communication and Interpersonal Skills: Excellent communication and interpersonal skills, with the ability to influence people and make generalizations, evaluations, or decisions based on sensory or judgmental criteria.
